Forms (need one to attend one)

To attend an offsite function with the church you must have both the attached froms on file. The Medical Form is a one time fill out form (unless med information changes), the other is specifc for each event. Feel free to print and bring it in. If we go over night more or more than 50 miles we will need to have the forms notarized. Jim will get that information to you as that need arises.
